RESTORE Act Advisory Committee Resolution Amendment Recommendation: Board to adopt amendments to resolution establishing the Bay County RESTORE Act Advisory Committee. 2. RESTORE Act Advisory Committee Appointment Recommendation: Board to ratify Dr. Stephen P. Leach, Associate Dean of FSU Panama City, as FSU Panama City's designee on the Bay County RESTORE Act Advisory Committee. Emergency Services 3. EMS Collection Services Recommendation: Board award the contract with the top ranked firm, DSG Collect to provide collection services for the EMS Division. Public Works 4. FY14 Annual Resurfacing Phase 2 Local Roads Recommendation: Board: 1) Award the bid for the FY14 Annual Resurfacing Phase 2 - Local Roads project to GAC Contractors, Inc., in the amount of $ 541,116.00 and, 2) Authorize contingency and testing funds in the amount of $32,500.00 (6%). (District I & III) Purchasing 5. EMS Medical Supplies Recommendation: Board to approve and authorize the Chairman to execute a one (1) year contract, subject to three additional one-year renewals with Bound Tree Medical to provide EMS Supplies to the Emergency Medical Services Division. 6. Amendment to Rental Cleaning of Linens for EMS Recommendation: Board authorize the Chairman to execute the First Amendment to the Contract with Crown Health Care Laundry Services, Inc. to include additional items for rental and cleaning of linens for the Emergency Medical Services (EMS) Division Ambulance Service. Clerks Reports 7. Revenues and Expenditures, Tax Certificate Refunds and/or Cancellations Recommendation: Board to acknowledge receipt of reports.
